Before you start — items to have on hand
Government photo ID (driver's license, NC ID card, US passport, or military ID).
VIN — copy from your registration card, NOT memory. Transposed digits are the #1 rejection cause.
Current address that matches your ID. If your ID is out of date, update it before filing MVR-4 or NCDMV will mail the title to the old address.
$21.50 — check or money order made to NCDMV. Cash accepted at LPA offices only.
Access to a notary — bank tellers (most NC banks notarize for customers free), UPS Store, FedEx Office, or a NCDMV LPA office.
Field-by-field walkthrough
Section 1 — Vehicle: enter VIN, year, make, model, body style, color. All exactly as they appear on your registration card.
Section 2 — Owner: legal name as it appears on your photo ID, current mailing address (where the title will be mailed), telephone, email (optional).
Section 3 — Reason for duplicate: check ONE box (lost, stolen, mutilated, never received, name change). 'Lost' is the most common.
Section 4 — Lienholder: if there is an active lien, enter the lender's exact business name and address. If no lien, write 'NONE'.
Section 5 — Affidavit: do NOT sign yet. Read the affidavit language fully.
Signature block: sign in the presence of a notary. The notary completes the notarial certificate, attaches their seal, and dates the document.
